Ureter, Bladder and Kidney Stone Treatment

Kidney stones are the tiny crystals which connect with each other to make a stone-like formation. They can be present in Kidney and ureter at the same time. Few very small stones get out of the body of their own with urine, but if they get blocked in the ureter or bladder then they can be very painful for the patient.

Depending on the size of the stone, it can also block the urine flow which is a medical emergency and should be treated immediately.

Symptoms:                

  • Pain traveling from lower rib area to lower back
  • Frequent Urinary Tract Infections (UTI)
  • Pain and burning during urination
  • Pain episodes come and go in a short or long intervals

The kidney stones treatment is commonly done by these methods:

  1. Ureterorenoscopic Lithotripsy with holmium Laser:

This technique is best for treating the stones in the ureter (the tube connecting the kidney and the urethra). The probe is passed into the ureter and the stone is broken with the help of the laser. The stone is broken down into the tiny sand like particles, after which they can flow out with urine painlessly. This Lithotripsy is the Minimally invasive technique for stone removal. It has the fastest recovery time. In most cases, only overnight hospitalization is required.

2. Percutaneous Nephrolithotomy (PCNL):

This procedure includes the passage of probe from the skin of the patient and not from the ureter. The big stone (more than 2 cm) is then broken down into the smaller ones and then removed. This procedure requires spinal or epidural anesthesia. Sometimes the ureteral stents or pipes are also placed inside the ureter to ensure the proper flow of urine till the healing takes place or the obstruction is removed. Mini-perc is the new technique that we use, which includes smaller puncture to remove kidney stones while doing PCNL. This allows much faster recovery and healing.

These ureteral stents are removed under local anesthesia after 5-10 days. They are not used in every patient.

3.Cystoscopic laser lithotripsy:

This procedure (also known as cystolitholapaxy) is for the removal of bladder stones. A cystoscope is inserted inside the urethra and then it reaches the bladder where the stone is trapped in a basket device and taken out from the urethral opening. In many cases, the urinary catheter is left in the bladder to ensure proper urine flow.

4.Retrograde Intrarenal Surgery (RIRS):

यूरेटर, ब्लैडर और किडनी स्टोन का इलाज:

गुर्दे की पथरी छोटे क्रिस्टल होते हैं जो एक दूसरे से जुड़कर पथरी जैसी संरचना बनाते हैं। वे एक ही समय में गुर्दे और मूत्रवाहिनी में मौजूद हो सकते हैं। यूरिन के साथ शरीर से कुछ बहुत ही छोटे-छोटे स्टोन निकल जाते हैं, लेकिन अगर ये यूरेटर या ब्लैडर में ब्लॉक हो जाते हैं तो ये मरीज के लिए काफी दर्दनाक हो सकते हैं।

पथरी के आकार के आधार पर, यह मूत्र प्रवाह को भी अवरुद्ध कर सकता है जो एक चिकित्सा आपात स्थिति है और इसका तुरंत इलाज किया जाना चाहिए।

लक्षण:

  • पसली के निचले हिस्से से पीठ के निचले हिस्से तक जाने वाला दर्द
  • बार-बार यूरिनरी ट्रैक्ट इन्फेक्शन (UTI)
  • पेशाब के दौरान दर्द और जलन
  • दर्द के एपिसोड छोटे या लंबे अंतराल में आते हैं और चले जाते हैं

गुर्दे की पथरी का उपचार आमतौर पर इन तरीकों से किया जाता है:

1. होल्मियम लेजर के साथ यूरेटेरोरेनोस्कोपिक लिथोट्रिप्सी:

यह तकनीक मूत्रवाहिनी (गुर्दे और मूत्रमार्ग को जोड़ने वाली नली) में पथरी के उपचार के लिए सर्वोत्तम है। जांच को मूत्रवाहिनी में भेज दिया जाता है और लेजर की मदद से पत्थर को तोड़ा जाता है। पत्थर छोटे-छोटे रेत जैसे कणों में टूट जाता है, जिसके बाद वे बिना दर्द के पेशाब के साथ बाहर निकल सकते हैं। यह लिथोट्रिप्सी स्टोन हटाने की मिनिमली इनवेसिव तकनीक है। इसमें सबसे तेज रिकवरी टाइम होता है। ज्यादातर मामलों में, केवल रात भर अस्पताल में भर्ती होने की आवश्यकता होती है।

2. परक्यूटेनियस नेफ्रोलिथोटॉमी (पीसीएनएल):

इस प्रक्रिया में रोगी की त्वचा से जांच का मार्ग शामिल है न कि मूत्रवाहिनी से। बड़े पत्थर (2 सेमी से अधिक) को फिर छोटे पत्थरों में तोड़ दिया जाता है और फिर हटा दिया जाता है। इस प्रक्रिया में स्पाइनल या एपिड्यूरल एनेस्थीसिया की आवश्यकता होती है। कभी-कभी उपचार होने तक या रुकावट दूर होने तक मूत्र का उचित प्रवाह सुनिश्चित करने के लिए मूत्रवाहिनी के अंदर मूत्रवाहिनी के स्टेंट या पाइप भी रखे जाते हैं। मिनी-पर्क नई तकनीक है जिसका हम उपयोग करते हैं, जिसमें पीसीएनएल करते समय गुर्दे की पथरी को हटाने के लिए छोटे पंचर शामिल हैं। यह बहुत तेजी से वसूली और उपचार की अनुमति देता है।

ये मूत्रवाहिनी स्टेंट 5-10 दिनों के बाद स्थानीय संज्ञाहरण के तहत हटा दिए जाते हैं। उनका उपयोग हर रोगी में नहीं किया जाता है।

3. सिस्टोस्कोपिक लेजर लिथोट्रिप्सी:

यह प्रक्रिया (जिसे सिस्टोलिथोलैपैक्सी भी कहा जाता है) मूत्राशय की पथरी को हटाने के लिए है। मूत्रमार्ग के अंदर एक सिस्टोस्कोप डाला जाता है और फिर यह मूत्राशय तक पहुंचता है जहां पत्थर एक टोकरी उपकरण में फंस जाता है और मूत्रमार्ग के उद्घाटन से बाहर निकाला जाता है। कई मामलों में, उचित मूत्र प्रवाह सुनिश्चित करने के लिए मूत्राशय में मूत्र कैथेटर छोड़ दिया जाता है।
